one try back in 04, (not a popular surname but it matched the person I was dating.... don't say it)
two tries on this av (which is the remix of the old one) but that's only because I entered the name to check, but didn't get it immediately ($10 fee), and the old name system permanently reserved it...
what I did have a bit of trouble with was actually coming up with this name... I went through the surnames looking for things that were sinple, had lots of potential for different styles, and could be twisted =) very much like me
I came up with Void Singer because it fit in so many ways... both personally, literally (since my av is 'in the void of cyber reality'), because I can't sing (a childhood dream) , and as a two paart sarcastic joke to some friends, since I'd give them advice they never took (to their own detriment) so it felt like I was 'talking into nothingness' (which is a slight agnostic rant also, even though I'm not). in the end it was loaded with more meaning than my original choice and I'm quite happy with it

NB: I am also running a poll on how many people you know with the same surname, it will be interesting to see how the survey results compare.... Took me forever to get a name when I first started way back in the days when they didn't change surnames and it seemed every reasonable name had been taken. All my alts since then, the alts I almost do not ever use now, were 1 choice and enter.

Maybe cuz it was a long time back, but I got a name I was happy with right off the bat. Found a last name that would work, put a first name with it, an poof - all done.
