Sorry, I'm not inclined to do your homework assignment for you.
Look, your questions show that you don't know the first thing about SL...for example, most people won't give you any Real Life information such as age and gender, especially since this is not an anonymous survey.
And you haven't thought your questions out very well. Do you really think you're going to find someone here, on the SL forums, who will answer "I have left Second Life"? Duh.
Log on. Go in world. Meet people, talk to them. DON'T hand them a form to fill out.
If you do want people to do a survey for you, at least have the courtesy to pay them for their time. My fee is $L250. Or $L2,500, if you want me to guarantee honest answers.

The Microsoft "Private Use" symbol \uF0A3 is a picture frame with an "X" in it, intended for use as a "missing image" icon. It's not a "checkbox".

Ooh, I've been waiting for a chance to post this!
"Similarly, some of the first educators and researchers in SL were a bit clumsy in their approach. There were early incidents in which Residents felt—rightly—that they were being spied upon or treated like guinea pigs rather than human beings. Imagine how you would feel if an entire class of students set up right next to your vacation hideaway, left trash lying around, logged your conversations, and posted them on the Internet with criticism and mean-spirited comments. This is exactly what happened to some early Residents. Things have improved greatly in the years since then, but sometimes SL Residents are still subject to that kind of inconsiderate behavior. For example, a constant stream of researchers (often students) posts on the Second Life official forums requesting that Residents please take their surveys. Most of these surveys include the same questions that have been asked over and over. Often they aren't spell-checked, or refer to SL as a game—a sure way to irritate a substantial number of Residents. Some Residents make a hobby of critiquing these surveys, pointing out how the questions and their wording make it clear that the researcher isn't familiar with Second Life. Some post outright that they are sick of badly planned surveys and suggest that the researchers log in and do their own research."

But no, according to http://www.alanwood.net/unicode/private_use_area.html#: "Symbol font from Microsoft "The characters from Symbol font can be seen at the following code points using a font viewer, but no Web browsers since Netscape 4.8 seem to be able to display them." I guess he wrote the survey in Word or Front Page and didn't actually bother proofreading it in a browser.

1- age: 19 2- gender: I don't see how this matters.
3- hobbies/interests: SL 4- occupation: Cryptologic Linguist (Soon, anyhow) In second life: 1- You are/were a SL member for how many months/years (duration)? Approximately 26 months, which equates to a little over two years. 2- Why did you enter SL? A good friend asked me to. 3- Are you still a member? No, of course not, that's why I'm on the SL Forums. Yes, I'm a member. If yes: 1. If you are a member in any social group/s, please write the name and the type/purpose of the social group/s: All of them are for helping people, really. Names and such don't matter. 2. Please pick the activities that you usually do in SL? (you can choose more than one) Build virtual objects or Sims
Did SL help you with your real life? If yes please write how: Yes, I'm happier. Go away. There you go, I've put as much effort into this as I feel comfortable doing, and I'm quite sure that it's either enough for you, or it isn't and you can't use it. I'm honestly hoping for the latter.
