My device didn't require an attachment. That seemed like cheating.
It did it from ground level using many scripts repeatedly calling llPushObject on the avatar. I had it in a big red button with a label that said "don't press this button". It's amusing to note that many people pressed it repeatedly, and complained when I removed it from my parcel.
In a typically loaded sim it shot you well over 100 million meters, and in an empty sim, well, I don't know just how high it went.

In my experimentation, I alternated between ground-based and attachment, generally believing that attachment could go further but ground-based was safer. A precursor to the final version of my device ended up forcing me out of SL for a month somehow

I haven't tried this myself. As such, I don't quite understand why your avatar deforms or attachments mess up when you are orbitted.

I'm not sure why avatars deform either, but in my observation, the avatar disappears past 1 million meters, and the sky turns black maybe at 10 billion meters. The client's builtin altimeter breaks at approx 2 billion meters, (MAXINT)

Not if you are pushed vertically. If you don't shoot perfectly vertically, though, the relative horizontal position of the avatar and the camera get trashed by rounding errors.
I could still see myself at 2 billion meters. Hm, I wonder what the difference was that caused me to appear to disappear. Speed? Too bad there's no way to really test, except using that libsl thingy, SLProxy or whatever. And that only goes up to MAX_INT

I only had the avatar completely disappear when I wasn't launched quite vertically, but of course that was in a much earlier version of SL.

There was a thread back in 2007? Some guy decided to see what would happen if his avatar just kept flying upward.
After ten hours his avatar was unrecognizable. He did post snapshots of his avatar as the experiment progressed. When he ended the flight, there was just this weird little lump of something left.
Somebody here might remember if the snapshots were posted online any where. I don't know if the original threads would be still archived here in RA. Maybe the poster is still around and will respond.

Just doing the math.... Assuming the Z-axis is expressed as a double-precision float, the theoretical maximum is 9,007,199,254,740,992 (just over 9 trillion meters).

You know, you can just teleport most of the way... change the max_val of "spin z" in skins/default/xui/whatever/floater_world_map.xml lol  If you make it too high, the viewer seems to not let you enter a Z value at all, so I went with 4294967295 Edit: I guess it's not most of the way, but I bet you can teleport to the top if the viewer didn't ruin it

To get 340.2 trillion trillion trillion meters up, I used 340.2 trillion trillion trillion units of force in 2 scripts, but that was in Havok 1.

Ive been upto 1,000,000 metres under Havok4 using llSetForce and ya took well over an hour. My avatar got its head and upper torso munted on the way up. Is quite funny when your eyes come out of your head =)
We were also able to get to MaxInt under Havok4 in a single jump for a while but LL closed that hole.
What I found interesting about way-up-there is that most things still work, like IM and Search and stuff. Can even set a LM but cant TP back to it which is a pity.
Also I like the suggestion about space. Would be really great if at about 4000m we were on the edge of space and when we looked down the world below curved away or something and above was just stars and inky space. Would be a great backdrop for UCC comets and asteroids and space stations and stuff like that. Homes in the heavens even. Be quite kool I think.

My "don't touch this" button pushed you MUCH harder than that in SL 1.7. To get to 2 billion meters in less than a second you have to travel faster than the speed of light.
