Here are some possibilities
- Misremembered - you may have made the friend request a LONG time ago, but that av hadn't logged in since then.
- Mis-click - usually the most likely, but to offer a friend request you have to make TWO mistakes: pie menu misclick and then hitting ENTER to send the request. But, if you're clicking and talking in rapid succession, it could happen.
- SL Bug - What? This is possible? OH NOES! Frankly, though, I find this particular one rather unlikely due to the number of interactions required.
- Exploit - this implies an SL bug, but one that someone is intentionally using for their own ends. This is rather unlikely, since there's not much benefit to being a friend. (Note: check that it doesn't have the "edit your objects" box checked! If so, it's most likely the next:
- Password compromise - someone logged in on your account by guessing your password.
I suggest you change your password in any case.

If i drop someone from my friends list, i prefer to do it when they are on online so that if they IM me with a "why?", i can give a long detailed reason to answer their question.
