I think most people who can have a bit of skills and can make a few unique things have the potential to support their gameplay. Beyond that, I don't know. I know I only make about $20-30 a month at best, so I'm not one to talk  It depends on how much land you have, of course.  However, I don't think it's possible for the majority to repay tier - because for them to do so, the minority who are putting money in would have to be putting in more money than the cost of tier without getting any land. Note that because there are so many people on SL, this isn't something that should discourage people though.

I run Lupinia Motor Company, a car company selling off-road vehicles and other designs I made. I originally started this business after building a replica of my boyfriend's VW Rabbit in SL, and decided other people might like cars. Thus far, that's the only model I carry that's based strictly on an RL design.
Since then, I've expanded a bit (I have a handful of vendors, a nice showroom on my land, plus a full inventory on SLEX), but it's always been for the fun of it rather than profit motivations. I don't see the point of doing anything in SL I don't enjoy doing.
Thus far, I've yet to turn a profit, but I think I'm getting closer. I haven't really kept track of my expenses.
