As I recall from doing Unreal and Unreal Tournament maps, you could do subtractive geometry and it would show up instantly.
BUT...
If you MOVE that subtractive brush (primitive in SL parlance), or move one of the brushes that was cut by it, you have to rebuild the whole level in order to see the change.
Linden Lab has already commented that handling arbitrary concave primitives would be difficult to implement on the server side. Maybe in a couple of years, when a 6GHz machine with 6GB RAM costs as much to rack as the current 1GHz/1GB RAM machines they are using today, it'll happen.

It's been a while since anyone posted in this thread. Can anyone give us an update? Has Linden said anything about this? Perhaps it's even implemented now, I don't know since I'm new...

So when people want to make a wall with a window in it, they basically create four slabs - one above the window, one below, and one on each side, leaving a window hole in the middle?

Ah, ok I get it. So if I want a wall with two windows, I place two hollowed slabs next to each other, and so on. Correct?
Hmm, come to think of it, is it possible to hollow in other places than dead center of an object?

That's right, and as far as I am aware only in the center. You can cheat making a door too by sinking a hollow slab in the ground making it look like a door!

Or cutting it. Also remember that you can "cut" any shape on any place of an object's face with an alpha texture.
