Linden Labs is doing a good job of hunting down the under age, and finding out if in fact they are under 18. I signed on one day to play not long after I had just joined SL. I got a message there was a problem and to contact the CS ppl. They needed/wanted proof I was over 18. I had to fax proof of my age to them, something along the lines of an offical document. With in mins of receiving my fax I was ALLOW back in to SL

A number of games (EQ, UO, etc.) actually sell "gametime" cards, usually found in your local software store (EB, Gamestop, etc.) Maybe Linden might want to look into doing something similar?
Although, given the very efficient download size of the client, I'm thinking they might not bother with hard media in the stores... so maybe that wouldn't be practical.
