I dont know how many transactions you have but the US banking system (visa in particular) flags as possible fraud after a certain amount of transactions going overseas it seems. They then put a hold on Linden/SL *only* (you can use your card for other transactions with different companies). What I have been trying to find out is how many transactions or dollar amount that is...the bank doesnt seem to have the exact information. You just know it when you get dinged. Again, this is for *high* transactions where the biling system is overseas is my understanding.
Linden Labs billing system is UK.

I'm in the UK too, so it's not just happening to Americans. My monthly tier isn't particularly high either. (Under 30 USD.) The two premium accounts that provide 1024m "free" tier are paid up annually so no monthly fee added on due to those.

From: someone American Express don't seem to understand *why* Second Life transactions are such a problem, but they do understand that those particular transactions *are* a problem, hence their advice to call on the 13th of every month to ensure my tier transaction goes through. Hopefully my formal complaint to AMEX will reveal the reason for the rejections. I expect that although the customer service reps at AMEX may not know the reason, but I'm pretty sure AMEX knows why. I look forward to hearing their official response but I'm pretty sure that LL is blacklisted as risky merchant. Even if SL hadn't been used for money laundering or credit card fraud, I imagine the accuracy of LLs billing has generated it own share of "double billing" or "incorrect billing" complaints at a rate higher than for most merchants.
