It depends which computer I'm on too.

On the best comp I have all the settings except draw distance set to 'full', with all options enabled. I have draw distance set to 256m. It generally works very smoothly.

On the older comp I have most things set to full too, but it can't do WindLight sky or water and I have draw distance at just 64m, but it's still slow and jerky.

In preferences I usually have avatar names set to show temporarily, then fade out, while I hide my own name on my screen. SL looks so much better when you don't see names hovering over peoples' heads all the time.

Re shadows - I have tried them and they look great but they do slow the frame rate down a lot and there are some faults like very hard-edged shadows on alpha textures and my avatar shadow breaks up after about 1 minute, with gaps in it.

I think my bottleneck is my mother board and chip; especially my motherboard.
I run 60-100 fps on Very High but I think I should get more; I need high fps for filming as the screen capture cuts fps by a third to a half. I vary my distance from 150 to 512 depending what I need. I don't turn on water reflections cause I don't see the difference.
I have particles on max cause I use particle effects a lot on what I film.
I haven't tried AA cause I hear it also makes a hit on the fps.
Emerald Viewer makes it look I have AA on anyway but I will give it a try.

Vista 64 bit
9600 Quad OC to 3.8Mgz (have gotten 4Mghz stable but the chip gets too hot)
Asus Striker Extreme II MB
nVidia GTX 295 2 Gig RAM slightly tweaked
8 Gig on board RAM OCd
