Clothes, use any graphics program. The Gimp, Photoshop, etc. Sculpties, use Blender or any of those other ones out there (Maya? not sure) What exactly do you want to do?

You would use a graphics program for that. There are lots of choices out there; wars have been fought over less. There is a whole forum here: Texturing Tips, which will help get you started. Definitely read the stickies; they are VITAL.

Download the clothing templates available on this site and elsewhere. Use the fabric pics as textures on the templates in Photoshop or your paint program of choice. You can use the beta grid to do free trial uploads of your clothing designs to check fit and seams. When you're satisfied, pay L$10 per item to upload the clothing articles to your main account. _____________________

I make original clothes in real life. How can I take my pics of them and upload them for use on SL? It is a lot like making a pattern from clothes in RL. You need to make "templates" that show the front, the back, the sleeves, etc etc etc _____________________

Well you can make sculpties in plenty of programs.. but I'm assuming you mean just normal prims.. I don't think there is anything that will let you properly build and then upload a full design into normal prims, however, I *think* there is an offline builder where you could build everything, and then just copy offsets, rotations, sizes, texture repeats, hollows..basically every aspect of the prims.. I'm not sure though, since I havent used it if it does exist.
I also think someone is working on something to do it completely, build offline and import.. but I can't remember where I heard that =/ |

Here's the url for Jeffrey Gomez's Blender script that lets you design in Blender using SL primitives and Thrash Epsilon's LSL script that let's you import them.
