Looking for script for suggestion box or guest book

Hi,

I'm looking for a simple (but modifiable) script to put in an object to use it as a type of suggestion box. I want user to be able to leave me a notecard in the box. And if it's not too hard, to have the box email me when notecards are submitted.

Does anyone have script like this they would be willing to share? Or know of an inexpensive one to purchase?
