I am using a menu dialog in a TP script. If "Ignore" is clicked and the prim containing the script is right click, a "0" shows up in the pie chart where the TP destination would otherwise appear. If that is clicked, the AV is sent flying off to the edge of the sim. While this is not a disaster, I am wondering if there is a way to avoid this continency.

The button of a dialog causes the agent to speak that button's text on the channel you've specified except for the "ignore" button which does nothing except dismiss the dialog. Thus, when someone clicks ignore they don't say anything.
If they don't say anything, your teleport script doesn't know where to go. It's destination isn't ever set.

You would have to give us more information. It sounds like you have something being set upon listening of the dialog. Thus, if the ignore button is pressed, nothing is ever said and the listen event is not triggered.
