I want to build an online survey or test, and submit the result to a server. I purchased this one solution (http://www.slexchange.com/modules.php?name=Marketplace&file=item&ItemID=85292) that sends the result to an email account, which I guess is good enough, but I would prefer to have a more user friendly input form. Ideally a big white screen where the user reads the questions and checks the answers (multiple choice), and when all is done, clicks submit to submit the result to a server/email. I guess I could have a web browser open (and submit the result in a query string or something).
Extra good would be if I could use sound files for some of the questions (click play to hear the sound and then pick the correct answer), and images.

Please advice on how to develop this, or even better if I could buy such a solution.

THe solution is relatively simple...

...a prim "board" that displays the question-texture (or floating text)

...a "button" for each multiple choice answer with the answer either: On a prim next to the button or on the button itself (making for a larger button size).

...Each result is sent to a MySQL Database through an llHTTPRequest that you can then interface through whatever web setup you choose.

I'd offer you my prices...but you'd probably run from them :-)

Yes, you can make sound files play for selected questions (or all questions).

My pricing varies dependent upon the resale-value of the work done (for myself), whether you'd mind me selling it myself, and the permissions/purposes you're looking to receive on the scripting. Essentially, fully custom work goes at my "real-life" programming price of US$100/hr (L$27,000/hr). It tiers down from there dependent on product permissions and resell-ability to as low as L$2700/hr.

Without knowing how extensive you want the web-interface to be, I can't really give you a firm estimate on time/price. However, the 'in-world' portion of the project would be a relatively simple task to accomplish and take no more than a couple hours.
