How do I use an animation application to position the second hand so that it would be on the handle in SL? And, how can the second hand "grab" the handle, when the hand and fingers can't be posed for a grip?

High end animation packages allow for IK setups that link the limbs to an object. So for your animation you could link the hands to the sword hilt and they would follow the sword.
I don't think applications like QAvimator or Poser have this option, but the rigs I built for 3ds Max and the Maya rig can be used to do this.
The hand positions can be set when the animation is uploaded to SL. There is an option for "two fists" or something like that. Set it and cross your fingers.

There's no fancy way to do this in SL. Just position the hands so that they are correct for holding the sword hilt, import the animation with a closed hand position, and edit the sword so that it fits into both hands. You will need to view, modify, repeat until satisfied. Use the beta grid to view your attempts to avoid spending lots of $L on upload fees.
