I had a similar problem to this and found that I was dropping items into other objects (the floor of my house mainly) rather than them rezzing ON the floor - when I discovered this and "opened" the floor in my normal rezzing spot I found all sorts of stuff.
Rezzing outdoor on real land is a bit safer.

The point has been being made for years. They just don't know how to fix it. My hypothesis has been that the client (and sims) were written never expecting the database operations to fail and so never check for error returns. As the databases are now operating at 5x or more of design capacity they must throw actions away. LL knows what the problem is, they just cannot or will not fix it.

Did you make the mistake of attempting to rez your objects on a shiny surface? For some reason I've never quite figured out rezzing seems to fail for me if I drag an object out of my inventory onto a shiny surface. Nowadays I always look for a non-shiny surface or terrain to rez object on.

It's rather rare, but it's worth taking a look at 0,0,0 on the sim as well, although I think that bug is related to object entry rather rezzing. Parts of one house were showing up there anyway.
Aside from that, it's a pain and it's not something you should be worrying about, but check how many are logged on and check the statistics for the sim you're on before you rez anything. I'll only rez something when there's low load and the sim is near-empty and it seems to work for me so far.

I was just explaining to a friend how to transfer anims from one AO to another, but she tried to rez her old one before I finished telling her to simply edit while attached, and it never rezzed. Going and camming down to 0,0,0 it was there so if you have a failed rez, cam under the land at the sim corner and it might simply have gone offworld

ghhhaaa ! Its happening again  This time it was diffrent , I had 2 parcels of land next to eachother ...sold 1 parcel and went to the other one to find missing items ... it must have been that my prim limit had over run to the other parcel and wene i sold i was left with the prim limit for only one plot ... but ... why was the stuff not returned to my lost and found folder .... this is soooo annoying
