Possibly you're using an attachment point that is not stable related to where you have the attachment?
For example, if you try to attach a prim nipple to the left and right pectoral attachment points (which seems at first glance like it makes perfectly good sense), it will 'float' a bit, sinking into and rising out of the chest, as the shoulder moves. The pectoral points would be better used for something positioned over the shoulder, like shoulder armor.
On the other hand you can link both nipples together and attach to the 'chest' attachment point, and it's rock solid stable.
Try a different attachment point, and it may help.

Also note that you can edit the position of your attachments. Just right click on the attachment and select "Edit" from the pie menu. Use the arrow handles to reposition your attachment relative to the attach point on your avatar. Since avatars can vary in shape and size, this is often necessary - especially with things like jewelry. Whenever I see a new resident with a necklace floating two inches above their neckline, I always send them a private message and let them know how they can fix that. It is common for new residents to just attach and not realize they can make adjustments.
