Ever since I started SL, in May of 2005, I have noticed that whenever I open a box containing more than 2 or 3 objects, I get a system message that states, "Items coming in too fast, automatic preview disabled for 10 seconds." My questions are 1. What is/was automatic preview, and what does it do for me when it's NOT disabled? 2. Why can't they fix it so it works all the time? 3. Should they explain this to us at some point, so noobs don't think they are doing something wrong (like I did the first few times I saw it?)

Automatic preview opens textures and notecards automatically when they are received. Uh, and I think LMs too.

There is a setting in Preferences to enable or disable it.

Getting whole lot of things at once, the system chokes, and so it automatically stops opening things on your screen. The items are still added to your inventory, however.

There is a limit, I think 10 items, before the disable preview kicks in.

The "items coming in too fast" throttle was put in because some griefing tools would flood an avatar with (usually hundreds or thousands of textures) which would overload and crash an avatar. Before the throttle was in place, someone could easily crash avatar's client program which is not good.

There's a variant of this tool that uses drop down menus instead of textures. Do you know of any way to defend against it?

If I remember correctly, muting the person will prevent them. I haven't tried this recently to be sure.
