A tattoo is basically an image with an alpha channel.
You create it your favourite image editting tool, then upload it to SL. You can then apply the tattoo using the "tattoo" slots in the Appearance Editor.

There is tutorials in content forum in textures that really helpful though I am still struggling with how to do the alpha thing with those tutorials.  |

If you have a store-bought skin that won`t do. So what you do is make a new clothing layer and add the tattoo onto that. |

Worst case scenario, you could even put it on a prim and attach it to the skin at an appropriate location. (You could then script it, making the tatoo changeable! Ooo! *makes a note in the big book of ideas*)
