Mostly, you get the right to own land and a premium stipend (for current registrations - 300L$). Premium accounts also come with some benefits. For example, once you go premium, your in world account shows "Payment info on File (or used)", which some business and land owners use to base their access lists off of.
It definitely pays to be premium.
