refer to topic for question... I still haven't been able to resolve my log in issue, and my ISP has no idea what is going on. However I think I have identified my problem in the following line:
INFO viewer: UserLoginLocationReply() Enabling 69.25.104.217:13002 with code 0
how is the code assigned?

Ok from my logs it looks like your not been assigned a circuit code, look slightly above for the following, i will xxx some of my number incase its user sensitive
2005-07-21T17:22:26Z INFO Requesting location... 2005-07-21T17:22:27Z INFO Circuit code 45XXX3 assigned. 2005-07-21T17:22:27Z INFO Received location from user server
I then get the proper login with circuit number as you've posted above

I didn't get a code assigned until I disconnect when SL stops loading at connecting to region.

I think you need to speak to a linden to see if this will cause the problem, as a number does get assigned but too late, see if you can get a specific server name and see if you have a good ping to it, considering where you are there may be packet loss to that particular server or there may be a bad routing table somewhere.
