How are some of these people editing the physical shape of thier AVs? I'm not talking about clothing or suits. I mean the actual shape is completely different. I saw a goblin walking around the other day. Complete with a hunched back, dragging knuckles, short stubby feet and big long nose and chin. I tried everything I could think of in the appearance tab and could not get anything even remotely close.
Can you actually edit your base avatare model?

Camille is right- most beyond-the-sliders appearance changes are attachments- the SL equivilant of a rubber nose or mask.
In the most extreme cases, the av will use an animation script to contort themself into a weird shape to fit into an unusual costume.
If you see an avitar that seems to be missing parts- a floating head, for example, s/he's probably masked her body with attachments that have an invisibility texture.
