In SL, click on the word "File" at the top left of the screen to open the File Menu. Then click where it says "Upload Image". In the dialog that pops up, navigate to the texture you want to upload, select the file, and click OK. The upload process will begin. (This will cost you L$10, by the way.)

Once you have the texture inworld, right click in your inventory, and follow the menu tree to create a new shirt (I believe it goes Create->Clothing->Shirt, but I'm not inworld right now to double check). Double click the shirt to put it on, and then go into appearance mode (right click on your avatar and select "Appearance" from the pie menu). Go to the shirt tab in the appearance editor, and click on the white box that says "Fabric". This will open the texture picker. Select the texture you just uploaded, and click OK. Your new shirt will now have your texture on it. Use the sliders to set the rest of the shirt's attributes, save, and exit appearance mode.

Voila, your new shirt.
