Poser isn't useful for making skins, except as a previewer for looking at your textures on the SL avatar model, and for creating animations that can be used in SL. Skinning is done in paint programs like Photoshop, or 3D paint programs like Bodypaint. Skin making is essentially the same as making clothing.

I started on a base skin using a program called AVPainter which can be purchased on SLEX. Its nice in that it can save as either Targa or Photoshop PSD. Basically, its a 3d program that lets you draw right on the 3d model. In itself its not so great, but the face it can save as as a layered PSD as well as be used to fix seams is extremely useful.
Basically i used my tablet to scratch out the muscle lines along the 3d model. Then on another layer, I scratched out (its like scetching with a pencil) the highlights. Then in Paint Shop Pro (Photoshop works fine) I took the layers and applied gaussian blur (about 5 points for the lines, something like 20-30 points for the highlights). I added a few effects in PSP to make it greyscale and not too high contrast (it takes mid tone overlays well) and then went back into AVPainter where I started cleaning seams. Then back to PSP to smooth out blotchy areas. Wash rince and repeat. I still have yet to draw the head details and smooth out the muscle lines, but you can see the basic idea.
I've been playing with Poser as well. It makes some interesting renders. I know some people have been able to export UVmaps from materials edited in Poser, but I have yet to find the right script to export them so they can be brought back into PSP or Photoshop. Still looking into it.
