I am doing that in Photoshop. I work a lot with paths when reating clothes. I take a layer where I create the shirt body, path my collar, change the path to a selection and fill it with the same color or a bit darker / lighter depending on the shirt. Then I create a new layer where I draw the shadows of the collar to make it look 3d'ish.

Then: New layer - highlights and another new layer for the shadows to create wrinkles. So you have everything on a seperate layer and can change it later if you have to.

Nat, I draw the collar using Multiply. I add a layer with dark color as a background. I then apply highlights using a Screen layer. I turn off the background layer and create a new layer. Using the Path tool I will outline the body of the garment and, add my color or fill. My collar will now work with any color garment.

I describe this technique in more detail in my Intermediate Clothing Tutorial, using the polo shirt. You can find it through search or look in the stickies.
