If your customer is using a default skin then your tattoo would be dropped into the tattoo slot on the skin tab of the appearance editor.

If your customer is using a custom skin they bought from someone else, then the skin textures are probably already using the tattoo slots, so the tattoo should be make into a clothing item that can be worn on the underwear layer. Or, you can talk to the person who made their skin and see if you can work out an arrangement where they'll add your tattoo to their skin texture on behalf of your customer.

Your best bet is to stick to putting the tattoo design on a underwear/shirt/jacket pattern. Be sure to download Chip's excellent patterns - (see sticky post in this forum) as they are absolutely the best patterns to work on. (SL pattern downloads are lame). Tattoos have to be made with alpha channels (see instructions also posted in this forum).

Giving your design to a skin artist is a little risky - you can loose that design and it can be copied by someone else - although most established skin artist are cool about preserving your design. But if you do this - I don't think you will be able to sell them to the general SL population as those good skin artist most like won't give you the permissions to sell their skins to the general populations.
