creating a 2 in one prim is tricky because you need to do what is known as vertex mashing.

pinching is one of these tricks.

however
the shape you wish looks like it can be easily done with a cube that is steched in one direction with some verts uselected fro mthe middle of the top and bottom.
this will leave a small border you can pinch down into a cube form again and properly scale to shape.

also

you will need to make a uvmap of the sculpty you wish to use so the texture is properly displayed on the texture

The problem is with keeping the edges on each plate. That will give texture jumps as the LOD changes. To avoid this you should use a 8 X , 8 Y and 0 Multires Cylinder to create the sculptie. Select every other column of vertice and scale them on x and y to 0 (s SHIFT z 0 ). Add a 2 level subsurf modifier with simple subdivision and bake the sculptie. You should then be able to use a normal texture with 4 X repeats. The only problem will be at the lowest LOD, where only 6 vertices are used as that can't represent the cross shape.

http://wiki.blender.org/index.php/Manual/Subsurf_Modifier

On the dialog picture there, those are the settings you need except for "Catmull Clark" which should read "Simple Subdiv"
