Open their profile and at bottom of profile there is s spot to drop inventory items into.
FYI, you can't be wearing the attachment when you send it, you have to take it off first.

You can't add an attachment to a PM here in the forums. However, you can include an image, just as you can in a forum message. copy the image's URL. Paste it into your message, bracketed like this: "  ", without the quotation marks. Using IM in Second Life, you can cut and paste a SLURL (Second Life URL address) into an IM. You can also cut and paste text. (CTRL+C and CTRL+V work more reliably than the Edit Menu Copy/Paste commands.) You can't add an attachment. If you create a notecard in SL, you can attach landmarks, other notecards, and textures by simply dragging them from your inventory and dropping them onto the open notecard. Save the notecard. Send the notecard and attachments to someone by dragging it from your inventory onto their avatar, or onto the 2nd Life tab of their Profile.

If you are talking about in-world, then Lindal's suggestion is perfect - just make a notecard and attach it to the notecard - send them the notecard.
I don't know how new you are, so forgive me if you already know this, but just in case:
Open inventory
choose create, then create New Note
It will pop up in your screen. Type whatever message you want, attach whatever attachment you want. Rename it at the top and click Save.
The note will still be named "New Note" in your inventory. If you can't find it, swith to "recent items" in your inventory and it will be there. Right click on it and rename it whatever you want.
Find your friend in "search" and pull up their profile. At the bottom, there will be a spot for dropping inventory on them. Drag the note from inventory and drop it there. They will get a note asking if they want to accept it. when they do, the notecard appears in their inventory, and they can open it, read it, open attachments, etc.
