When I'm changing skins multiple times, deciding on which skin to wear, the skin that I click on in my inventory doesn't show that I'm wearing it, it shows I'm still wearing the previous skin. Then I go into appearance and immediately see that my avatar is solid black. I try the Ctrl-Alt-R several times, but always need to relog to fix it. This happens with clothing also. My friends see that I have changed my clothing and also my skin but not me.
Why does this happen?

There are several possible causes, but let me ask one question: are you using two monitors? If so, try moving SL to the main monitor.

switch to and from appearance mode
rebake
clear cache, relog to a new region
those steps are pretty much guaranteed to fix it if it's not a problem of corruption in the asset database. the change region forces the region to request the file from the asset DB instead of from a user that may have cached the bad version from your borked local cache.
