I got a lab coat yesterday that is a shirt or jacket layer plus a skirt layer. There are gaps between the top of the skirt layer and the rest of the garment. Is there some way to get rid of this? I have other garments with jacket/skirt layer combo and I don't remember seeing this before. I also notice now that my butt sticks through the skirt a little. Here's an image:

Unavoidable. One of the many examples of how poor the Second Life avatar and clothing mesh really is.
You can give the skirt a little more poof or bustle to keep your butt from peeking through. But that is why most skirts come with matching "glitch pants".
The only thing to do for the loose fit at the waistline is to wear a prim belt. Absolutely nothing you can do will close that gap at either the skirt or jacket layer waist joint.

I would download and capture both patterns and use a lower body mesh to make a smoother jacket bottom.
This is why I've been experimenting with "barrel dresses." There is no one piece dress mesh.
By the way, I thought "glitch pants" were called "dance pants." I've gotten used to including them as part of the outfit.
Another way to get a nice smooth skirt/shirt fit is use the undershirt as the shirt part of the outfit. Again this means fitting the texture to something else.

Aw poo.  I'll have to check the permissions on the outfit are and see if I can make the whole thing out of the jacket. Thanks for your help!

I find the standard outfits in SL very buggy, and I haven't seen much improvement yet. Luckily there are tons of custom clothing, where the artists actually took time to make it look good.
I find them in Freebie houses. Some are really good for something that is free of charge.

a couple of suggestions, which might not work but will only take a minute to try:
1 - make your backside a bit smaller. I use a different small-bummed shape for any outfit involving a system skirt.
2 - if the top part is on the shirt layer and is modable, try making it a bit baggy.
