It means you didn't get logged out properly-maybe your connection isn't very stable...I only get this if I crash due to a cut connection.

like Alyx I usually see it when I do a speed relog after a crash, but have caught it once or twice after trying to login and getting "despite our best efforts something has gone wrong", then trying to log in again and got OP's message...
oh and speed relogging is acheived by adding the command line parameters -noprobe -login First Last Password to your shortcut (mac users need to ad them to the arguments file) and then selecting a hot key for the shortcut (it's in the properties for the shortcut, dunno the mac equivalent) crash, hit hot key combo (mines ctrl+alt+s), relog takes seconds... skips right past the login screen... often faster than the sim can finish logging you out =) _____________________

Personally, I put the -multiple before the -login (for convenience, so I can quickly change the account), and that has always worked fine for me.
