If you make another avatar, an alt, it's a NEW account. Unfortunately, you can only pick from the list of last names SL offers. Someone mentioned that their were different last name options when you opened an account through OnRez, not sure if that's correct, though. If you absolutely want to use the same last name as you presently have, you have to take the same steps as before. If your surname isn't on the list anymore, you're out of luck.
