Since yesterday 90% of the teleports on my client will fail and the "sorry the system was unable to process your teleport request in a timely fashion, please try again in a few minutes" message will pop up. It seems to happen the most if I try to teleport home via world -> teleport home, I can rarely if ever get home these two days; but it happens very often with any other location also. Is this a confirmed bug or something?

Wasn't server updates stated yesterday? That is a common problem when they do massive server updates. Seems it might be this:
[COMPLETED] Unscheduled Maintenance Posted by Status Desk on September 4th, 2009 at 06:28 pm PDT [COMPLETED 6:21PM PDT] Minor maintenence that has occured is now complete however the issue is not resolved so we will be doing more maintenance at some point and we will update this blog as more information is available

How many HUDs or prim attachments do you have on?
Try taking some off before teleporting and see if it helps.

Oh I see I didn't know there was a server update. I didn't find it on the SL status page, unless you mean the rolling restart? From: Vance Adder How many HUDs or prim attachments do you have on?
Try taking some off before teleporting and see if it helps. I'm wearing lots of sculpted jewelry since I model the store's clothing, but I've worn way more before and it has never happened. As for HUDs the only two I have on are the AO and the couples cuddle invite.

Do the rolling restarts affect regions or avatars? Because other residents on the same land can teleport there no problem... I'm the only one I know who can't.

I wonder if this is related to mono scripts in prim attachments.
If this only started yesterday it's probably just the server acting up.

Fly up to above 200-300m or wherever it is not busy and do the TP again - this almost always works for me.

If I understand what I read, they may be a big source of lag when they first enter/leave a sim, much moreso than the old technology. Not everything uses it, but a lot does now. Most times, in busy places, I have 1 hud (my ao) and all my prim clothing attachments have the resize scripts deleted. Other people I've asked turn out to have 4 huds attached and a plethora of scripted attachments. It's not hard to wonder why busy places are impossible to move around. Just something to be cognizant of if it's not the problem.

Certainly having a load of scripts running when you tp doesn't help; to oversimplify lots, when you tp from one sim to another, the sim has to hand over all the information about your avatar, including all the info about all the scripts that are running -- what state they are in and what data is stored and so on -- and the more you have, the longer the process takes, and the longer it takes the more likely it is to time out.
Having said that, it can be your particular connection that's causing problems. I once asked Live Chat for assistance because I couldn't tp anywhere for about two days, no matter what I did, while friends were able to come and go as they pleased (so it wasn't SL or the sim being difficult). Spike Linden (who knows everything) suggested trying rebooting my modem, to force a new connection, as something that wasn't infallible but would fix the problem more often than not, and he was right.. it usually (though not always) fixes the problem for me.
