I believe on the desktop itself, you use Control-click as opposed to Command-click to simulate a right-click.
In SL, you use Command-click to right-click.

Command key is the 'apple' key and thats the one you use to Right Click

But seriously, tell your friend to go to a computer store and buy a nice normal two button USB mouse with a scroll wheel. Kensington and Logitech have been good brands for me. It will be a big help, not just for SL, but for almost everything on the Mac.

Depends how recent the Mac is as well. If it has one of the new "Mighty Mouse" things, it can right-click. The confusing thing is that it's a single piece of white, rounded plastic with a little roller-ball in the middle at the top, but if you lean down on the left side, it triggers a left click, and a right click if you lean down on the right. Takes a lot of getting used to though, and I generally dislike the mice with a vengeance, right-clicking is weird to do, and you can't hold right-click and then left-click while it's still held down (same bit of a plastic!). There's stylish and functional, and stylish to the detriment of functionality. Unfortunately Apple failed and got the latter with their "Mighty" mouse

If your friend is trying to right click on the desktop of a Mac to change display settings, they're not there. They're in System Preferences off the Apple menu.

There are plenty of really good 3-button USB mice that are Mac Compatable. I highly recommend buying one. The Logitec LX-3 is one I just bought for my new system.

As someone else points out, the Mighty Mouse can be configured in System Preferences to do a right-click.
Mighty Mouse is actually a 4-button mouse. The left side, right side, roller ball, and side squeeze can each be assigned some action.

Also want to point out on newish Mac notebooks you can place two fingers on the trackpad then click and you get a "right-click" if you enable this in System Preferences. You can also scroll with two fingers. I've gotten to the point that I miss this when I'm not at my MacBook Pro or PowerBook G4.
