Had the same issue because I'm using an nVidia GeForce 8800 card and just updated the drivers. Apparently the most recent drivers don't work with SL. I corrected the problem by downloading the driver 175.?? something, thanks to the help of the kind folks on the forums and at the Cartel. Mine seems to work okay now. Could this be your problem?

Have a look at this JIRA... http://jira.secondlife.com/browse/VWR-7957 Probably what you are experiencing. You can look for older nVidia drivers here... http://www.nvidia.com/Download/Find.aspx?lang=en-us The 175 drivers are listed there when I search using my card specs. I also see that there is now a 178 version driver available. will be interesting to see if that has the same problem or if it is fixed with that.

this is an old bug that seems to be reappearing inconsistently.. I had it on the pants and shirt yesterday took 6 re bakes and a change of pants to get rid of it. (I use the RC clients) and have noticed it more and more the last RCs from 1.20 as well as now 1.21RC3 there was a Jira on it and can't find it I guess since its not easy to re produce... EDIT ^ there it is.. and I use 169.1 Nvidia Drivers the 175 destroy to much stuff

From: Kirby Burnstein I also see that there is now a 178 version driver available. will be interesting to see if that has the same problem or if it is fixed with that. One of the versions I had before the most recent update was causing me to crash repeatedly whenever another avatar approached too closely -- at least some knowledgeable forum folk thought so. Upgrading to the newest driver stopped that, but then I had pants going all the way to there (or even to There). So no - I'd bet the 178 release is not the cure. 175 seems to be working.

Well, I'm way behind the curve on driver releases. I'm still using 169.25 on an nVidia 8600GT. I have no issues at all with SL.
I'm going to toss out my spiel about updating drivers again. If there is no real evidence that your driver needs updating, don't do it. Yes, a common suggestion for almost anything relating to SL problems is to "update your drivers". Good advice if your driver is outdated enough to be the cause, but often people update their drivers for issues that really don't relate to the problem being graphics. Losing connection happens often and is very often discribed as a "crash"........updating any driver for that is probably wasted time and possibly compounding the problem by introducing another, completely different, set of problems. Don't get me wrong, you do need a driver that will handle all that SL throws at it.....but many "older" drivers will do that just fine. Some people update their drivers as soon as they come out thinking they are heading off potential problems........that update often is the cause of the problem. That new driver does not function properly in the SL environment.
And when it comes time to update your drivers you need to do it correctly. Don't use any online "auto installations".......save to your computer and install from there, manually. Don't install any driver over an existing driver......uninstall the older version first. If you are an ATI user then use ATI's driver cleaner utility........nVidia does not have that feature so uninstall is your option (as is the rest of the video drivers). There are driver cleaner utilities available on the internet (some are free) but I've never used one so I can't vouche one way or the other......the uninstall has worked for me quite well for a number of years.
