I have a friend with a disability that means they are physically incapable of pressing multiple keys. Is there any way to open the Advanced menu apart from Ctrl-Alt-D?

None that I'm aware of. I think the Release Clients have it open by default. But apart from that the only way for this to happen is if someone does it for her. But once opened it stays open.

just have a care giver open it for them, once its opened it stays open at the top of the tool bar, till deliberatly closed

Sticky Keys

If your friend is using Windows, tell him/her to enable Sticky Keys. It's a feature Windows has had for many years now, specifically for disabled people just like your friend. When it's turned on, modifier keys such as shfit, ctrl, alt, and Start, remain active until another key is pressed and released. So all your friend would have to do is press ctrl, alt, and D in sequence, instead of all at once.
To enable Sticky Keys, simply press the left shift key five times, and then click OK. To disable it, press left-shift five times again.

Yes, I'd think "sticky keys" would be pretty important for that person to know for other reasons, too. Another option could be to edit the settings.xml file (or settings_releasecandidate.xml, if using the RC) in the application data folder (location varies by operating system), and insert this somewhere: From: someone <key>UseDebugMenus</key> <map> <key>Comment</key> <string>Turns on "Debug" menu</string> <key>Type</key> <string>Boolean</string> <key>Value</key> <integer>1</integer> </map>
(But "sticky keys" is better.)
