Under the Help menu, click Submit a Ticket. Of course, this won't actually take you to the Submit a Ticket page, it takes you to the Support page. Scroll down and under Additional resources you'll see a link for Incident Reports.

The "Help" text at the top is a clickable link (so are the other headings). YOu can get right to that page from there, you don't need to drill down through those black menus.
