This problem started with the Universal Binary version of SL, but since it also manifests under the PPC client running under Rosetta, I figured I'd post it here as well.
The problem: No sound at all in SL. No UI effects, music, nothing.
Sound in all other applications seems to work fine. I saw this in the log when I start Second Life (and before I log in)
2006-04-24T19:58:19Z INFO: LLAudioEngine_FMOD::init() initializing FMOD 2006-04-24T19:58:20Z WARNING: Error initializing FMOD: Error initializing output device. 2006-04-24T19:58:20Z 2006-04-24T19:58:20Z WARNING: Unable to initialize audio engine
This happens with the latest beta client (for the beta grid) as well.
I don't believe I've changed anything on this machine, and it was working fine yesterday.
Any ideas?

Fixed!
04-24-2006 14:54
Problem fixed! After much poking around on the Internet, I learned what happened. Turns out Audacity reset the default output sample rate on the machine to 96000Hz! FMOD, the sound output plugin used by Second Life, does not like this. Turns out the solution is right here: http://docs.info.apple.com/article.html?artnum=300832As soon as I set the default output sample rate to 44100Hz, sound started working again! This seems to be a general Mac issue and not related to the universal binary beta client. -Z

From: Brent Linden Bump This just helped me out! Wow, I helped an almighty Linden! Perhaps add code in the Mac client to reset the output to 44.1 on startup so this isn't an issue? Of course, not being a developer, this could be thousands of times easier to say than do. -Z

Can't fix the problem on my Mac Book
06-11-2006 16:45
My sound also stopped working!
Also when I ran audacity.
However on the Mac Book I am using, the stated fix from apple does not work.
The Midi Utility claims I have no output to adjust!
anyone else have an idea how to reset my output sample rate?

fixed loss of SL sound on MACBOOK
06-13-2006 14:43
I had to go to the genius bar at my local apple store.
You can also restore sound by opening garage band, then starting a new file
garage band will resert the sample rate to 44.1 KHZ restoring the output sound.

If anyone can figure this out, I would be eternally grateful. Midi input is OK Output is not available!!! Everything else seems to work fine, except sound in SL. At least with this honking new iMac with 2gbs I am not crashing all the time, but would I love to hear my favorite live performers. Thanks for the help.

Quicktime preferences were set on my new iMac to disallow any other audio EXCEPT from QuickTime. Works fine now. Thanks for the tip.

Mac Sound
06-16-2008 07:45
Hi
I've also suffered from this loss of sound in SL. When I use a different application that changes the sample and bit rates...SL then has absolutely no sound. I have tried the Audio/midi box change the rate fix...but I still have no sound in SL. The only that seems to fix this for me is rebooting about 10 times!
I'm using an Intel Mac 2.16 Ghz duo core with Mac OS X 10.4.11
This issue is especially annoy as I am a musician and often change sound cards and setting in the course of my work.
Any comprehensive help gratefully received!!
