if SL is an 18 and over game (excluding teen sl of course) then why do we have PG sims?

Because there would be no enforcable barrier for people that don't want a porn shop next door.

But on that note, the PG/M situation needs some major work. :p

See rejected voting proposal number 279

from the secondlife.com/vote pages, boldface italics added:

Prop: 279 - All Sims MATURE
[...]
Feature Detail: I think since SL is supposed to be a mature game and since you have to be 18 years or older to play ALL sims should be MATURE.

Linden comments: The PG designation shouldn't be taken literally to mean 'parental guidance'. We borrowed from the movie rating system in using that rating name, but its real value is in designating a space in SL where profanity, nudity, and adult content won't be present. Only about 20% of the land is designated PG, and there are SL residents who appreciate having that option.

I'm still reporting 14 year olds to LL.
