How come sometimes, even when I'm on the ground, the mini-map still says there are people lower than me, even when I'm right on top of them?

I've noticed that on occassion. Take a look at the sim FPS and time dialation.......it usually is off the norm when I see that happen. That would indicate it has somthing to do with serverside lag.

HI Sir Corn, the protocol needs an update to show us higher altitude. When LL fixes this it will only be fixed in new clients. Old ones will still show the funny numbers.

I noticed this start happening I think when the 1.22 viewer came out or possibly it was 1.21. Avatars that are much higher than you show up as below you and vice versa. Just something else that LL broke.

That is funny! There was no conspiracy that broke it. Nobody cared before Havok4, only flying avatars showed wrong. Building can go higher now so it is more easy to see the old limit.
