What most likely happened is you offered someone inventory while they were offline, and they finally logged in. However between the time you offered and they accepted, the object got deleted or change or whatnot. I've had this happen a few times when giving items to people who hadn't logged on in ages, and a few weeks or more later they show up and I've got a new version or something.

I would hazard a different guess. Did you try and drag an item from an objects inventory onto them or their profile? Or was it from your inventory?

I droped a note card from my inventory onto someone's profile.
Then i made some changes to the same item about 12 hours later and give it a new title, but did not give them a copy of the updated version. _____________________

If they were offline when you dropped the card on them, and did not log on until after you changed it then that would probably explain it. Modifying notecards gives them a new UUID in the database. The old one that you dropped on their card would no longer be valid when they log in and actually recieve the item.
Dragging from object inventory to an av will always give the message no matter their online state. _____________________

The UUID or key is a 128bit value that uniquely identifies a particular resource, be it object, script, texture, sound, avatar, etc. One of the context-menu items for objects in inventory is "Copy UUID" or something like that if you want to see one. They look something like "228136b0-8bd3-11d0-b4ef-00a0c9138ca4" in "human readable" form.
